Please note:

  • The registration form ánd information form for your new GP must be filled in completely.
  • We must identify you to complete your registration. To that end, please sign this form after completing it and hand it in at the reception of your new health centre.
  • Remember to bring a valid proof of insurance and photo ID (driver’s license, passport or ID card).
  • If you are registering children under sixteen, we also need to see a valid proof of insurance and photo ID (driver’s license, passport or ID card) for them.

Do you prefer to receive the registration forms at your private address?

Please fill in the online form. We will then arrange that you will receive registration forms at home.
